Missing or Damaged Equipment
Any equipment
found to be missing or irreparably damaged will be replaced by the Center or
the appropriate monies refunded to DVR.
Equipment with a unit purchase price of $300 or more that is lost,
stolen, or broken will be reported to DVR.
Proper documentation will be required, such as police or accounting
reports. Depreciation computation for
replacement or reimbursement to DVR on equipment with a purchase price of from
$300 to $1,000 will be straight line over the useful life of the equipment.
Disposition Requirements
If the Center should cease operation, the equipment purchased with independent living grant funds will be disposed of in accordance with the rules and regulations of the DVR. Disposal will involve one or more of the following at the discretion of the state agency.
1. The equipment
will be donated to another Center supported by DVR independent living grants or
utilized by DVR or another Center, agency or organization having purposes consistent
with the purposes of the Establishment Grant Program.
2. The equipment
will be sold and the proceeds given to another Center approved by DVR.
3. The equipment will be sold and the proceeds returned to DVR in the same proportion as the original independent living grant.
